• Fill out one Coach Evaluation per child; if you have more than one child who played in the  New England Storm program this season, please return to this page to fill out additional Evaluations
• All evaluations are STRICTLY CONFIDENTIAL and you DO NOT HAVE TO PROVIDE YOUR NAME
• We encourage you to provide your name IF you'd like to get a follow-up phone call to discuss any issues you may have - we find that this is really the best way for us to fully understand your issues.
• Coach Evaluations are sent directly to Rick Gorman for review; if it's necessary to address a coaching situation, any feedback given to coaches is done WITHOUT USING NAMES (if supplied) OR SPECIFIC COMMENTS ... only general feedback is discussed with the coach.
